RECORDS MANAGEMENT

The  Vandalia Fire  Division  Records Management is responsible  for  providing  EMS and  Fire Reports while ensuring that all reports released are  in compliance  with  the  Freedom  of Information  Act  as  well 
as
HIPAA guidelines, and confidentiality policies.

     EMS Reports (Medical Services Rendered)

               Submit a written letter of  request for record or the completed  form provided  by the Department.
               You are not required to put the request in writing; however, by doing so it can assist in accessing
               and defining the information requested.

               Patient - requires photo identification.
              
Parent may pick up a report with birth certificate (of child) and picture ID (of parent).
               Legal representation should provide a release of information signed by the patient.

     FIRE Reports (No Medical Services Rendered)

               Submit a written letter of request  for record or  the completed form  provided by  the Department.
               You are not required to put the request in writing; however, by doing so it can assist in accessing
               and defining the information requested. 
              
Fire  reports are  public record,  and anyone  can request  a fire  report with  the exception  of  an
               associated  casualty  report  -  this  is  subject  to  the  same  criteria  as  an EMS  report or a fire
               report that is involved in a criminal investigation.
 
               Fire reports will be provided as soon as the investigation has been completed and recorded.

     Any of the following information is helpful to process a request:

Records  management  is  located  in the administration office of  Fire  Station #1,  8705  Peters  Pike, Vandalia, Ohio 45377. Business hours are Monday-Friday 8:30 a.m. - 4:30 p.m. The main phone number is 937-898-2261.

Note:   Records are not always  immediately  accessible. The Fire Division strives to  provide information within a reasonable period of time.    Mail requests should be expected to take 7-10 days.
